Forms to complete before your Adventure

For all our Bushcraft activities please complete and return to us, via email or printed and handed to us on the day of your activity.

To save paper and ink you must read and understand the forms below, then copy and paste the summary below and email your compliance to us.

By doing so you are agreeing to all the information contained in the forms.

"I am sending this email to Spirited Adventure to confirm my responsibilities and compliance with the forms for the woodland activities.

I have read and understood the forms below. (delete firearms if not air rifle shooting)

Medical - please make S.A. staff aware of any medical condition, allergy etc that you think we need to be aware of while you are spending time in the woods.

(Spirited Adventure has a duty of care to you but we can't help/avoid if we are not made aware of specifics)

You must provide an emergency contact details. (a relative or  friend not with you on the day)

You must sign and date to agree with all the above information.
